Ironworker LMS New User Request
Form is intended to be filled out by training center personnel for each member. All information entered below is for your student (including the initial email address request).

Enter the student's email and other requested info below. Every student added to the LMS must have a unique email address, no exceptions.

STUDENT WARNING: This form is made available to be filled out by LOCAL LMS ADMINISTRATORS ONLY. If you are an Ironworker seeking to join the LMS, please contact your local training center.

NAME ENTRY WARNING: Names need to be entered as a single word. Character spaces within a name result in accounts where users cannot see the content of their assignments. Please do not add suffixes or nicknames to name fields. A Suffix field is provided for entering suffixes. Hyphenated last names are acceptable if a hyphen is used between names (and there are no spaces between characters).

Local Cohort Identifier (optional)
Local Cohort Identifier entries can be filtered against when running reports. For example, 2022-2026-08 could be used for a group of students entering your program in August 2022 and graduating in 2026. Or, if you manage your students quarterly, you might enter 2023-1 for students starting your program in the first quarter of 2023. Use what works for you.

Enter the name of a local enrollment group you would like this member added (be precise, even an extra character can mess this up/create a new group). If the group entered does not exist, the system will create a new one. (Always start group names with your 3-digit local designation.)
